Lung Cancer Screening and Diagnosis: What Every Patient Should Understand
The report comes back from a CT scan ordered for something else — a follow-up on a cough, a pre-operative evaluation, an imaging study for an unrelated complaint — and somewhere in the findings section is a phrase the patient wasn't expecting: pulmonary nodule identified.
For most patients, those two words initiate a particular kind of fear. The mind goes immediately to cancer. The fear is understandable, and it is also, in the majority of cases, disproportionate to the actual finding. Most lung nodules are benign — the remnants of prior infections, small areas of scarring, calcified granulomas from decades-old exposures. The vast majority will never cause harm.
But some lung nodules are early-stage lung cancers. And for the patients whose nodules fall into that category, what happens next — how the nodule is evaluated, whether the right specialists are involved, how quickly and accurately a diagnosis is reached — can be among the most consequential medical experiences of their lives.
Understanding the difference between a finding that warrants monitoring and one that warrants urgent evaluation, what the workup process should look like, and when the expertise of a thoracic specialist at a major academic center is genuinely needed is what this article is about.
Lung Cancer Screening: Who Should Be Screened and Why
Lung cancer is the leading cause of cancer death in the United States, responsible for more deaths annually than breast, prostate, and colorectal cancer combined. It kills more people than it should, in large part because it is frequently diagnosed at an advanced stage — when it has already spread and curative treatment is no longer possible.
Low-dose CT screening changes this equation for high-risk individuals. The National Lung Screening Trial, a landmark study involving more than 53,000 participants, found that annual low-dose CT screening in high-risk adults reduced lung cancer mortality by 20% compared to chest X-ray screening. Subsequent studies have confirmed and expanded these findings.
Current guidelines from the United States Preventive Services Task Force recommend annual low-dose CT lung cancer screening for adults between 50 and 80 years of age who have a 20 pack-year smoking history and currently smoke or have quit within the past 15 years. These criteria identify the population at highest risk for lung cancer — those for whom the benefit of early detection clearly outweighs the risks of screening, including radiation exposure and the downstream consequences of incidental findings.
Patients who meet these criteria should be discussing lung cancer screening with their primary care physician. Many eligible patients are not being screened — either because the conversation hasn't happened or because the patient doesn't know they qualify. Given the mortality reduction associated with screening, this gap represents a meaningful missed opportunity for early detection.
For patients who don't meet the formal screening criteria but have other risk factors — significant occupational exposures (asbestos, radon, arsenic, chromium), a personal or family history of lung cancer, or other significant pulmonary conditions — discussing the appropriateness of screening with a physician is reasonable even outside the formal guidelines.
What Happens When a Nodule Is Found
A lung nodule is a small, rounded area of density visible on imaging. The majority of lung nodules detected through screening or incidentally on imaging performed for other purposes are benign — but the characteristics of the nodule and the patient's risk factors determine how it should be managed.
Several features guide the clinical assessment of a lung nodule. Size is the most important: smaller nodules (under 6 millimeters) carry a very low risk of malignancy and typically require no follow-up in low-risk patients. Larger nodules — particularly those above 8 millimeters — carry a higher risk that warrants closer evaluation. Shape and density matter as well: solid nodules behave differently than ground-glass or part-solid nodules, which have their own risk profiles and follow-up recommendations. Growth over time — whether a nodule has increased in size between serial scans — is one of the most important indicators of potential malignancy.
Established guidelines from the Fleischner Society — the professional organization whose recommendations guide the management of lung nodules — provide specific follow-up recommendations based on nodule size, type, and patient risk factors. These guidelines are the starting point for appropriate nodule management, and patients whose nodule has been identified should ask their physician specifically how the recommended management compares to these guidelines.
What many patients don't know is that not all nodule management is equivalent. A patient whose nodule is being followed with appropriate serial imaging at appropriate intervals, by a physician familiar with the current guidelines, is in a different position than one whose nodule was noted and then effectively forgotten. Patients should ask specifically: what is the follow-up plan for this nodule, what are we looking for in the follow-up imaging, and under what circumstances would the plan change?
The Workup When Lung Cancer Is Suspected
When a lung nodule has characteristics that raise concern for malignancy — size above the threshold for watchful waiting, growth on serial imaging, irregular shape, certain density patterns in the context of clinical risk factors — further evaluation is warranted. The workup process that follows is consequential, and its quality varies.
PET scan (positron emission tomography) evaluates metabolic activity in the nodule and in other parts of the body. Metabolically active nodules — those that take up the PET tracer — are more likely to be malignant. PET scanning also evaluates whether there are other areas of metabolic activity that could indicate spread of disease, which affects staging.
Bronchoscopy — a procedure in which a thin, flexible scope is passed through the airways to visualize and sample tissue from the lungs — is one approach to obtaining a tissue diagnosis for central lesions accessible by this route. Newer technologies, including navigational bronchoscopy, allow access to more peripheral nodules that traditional bronchoscopy couldn't reach.
CT-guided biopsy uses CT imaging to guide a needle through the chest wall to sample a peripheral nodule. This approach is often used for nodules not accessible by bronchoscopy. It carries a small risk of pneumothorax — air leaking into the space around the lung — that is manageable in most cases.
Surgical biopsy — through video-assisted thoracoscopic surgery (VATS) or, in some cases, open thoracotomy — may be both diagnostic and therapeutic: the nodule is removed and examined, and if it is cancer, the surgery may also represent definitive treatment for early-stage disease.
The choice between these approaches depends on the size and location of the nodule, the patient's overall health and pulmonary function, and the expertise available at the treating institution. This is an area where the difference between a thoracic specialist at a high-volume center and a general surgeon at a community hospital can be significant — both in terms of technical capability and in terms of clinical judgment about which approach is appropriate.
Types of Lung Cancer and How Treatment Varies
Lung cancer is broadly divided into two main categories that drive treatment decisions in fundamentally different ways.
Non-small cell lung cancer accounts for approximately 85% of lung cancers and includes three main subtypes: adenocarcinoma (the most common subtype, and the one most often found in non-smokers and in peripheral nodules), squamous cell carcinoma (more centrally located and more strongly associated with smoking history), and large cell carcinoma. The treatment landscape for non-small cell lung cancer has been transformed over the past decade by the identification of specific molecular drivers — genetic mutations in the tumor that predict response to targeted therapies.
Molecular testing of the tumor tissue — evaluating for mutations in genes including EGFR, ALK, ROS1, KRAS, BRAF, MET, and others — has become a standard and essential component of the diagnostic workup for non-small cell lung cancer. The presence of specific mutations determines eligibility for targeted therapies that are often more effective and better tolerated than conventional chemotherapy for patients whose tumors carry them. PD-L1 expression testing determines eligibility for immunotherapy agents.
These tests must be performed on the tumor tissue, and their results meaningfully change treatment recommendations. A patient whose workup did not include comprehensive molecular testing has an incomplete workup — regardless of whether a tissue diagnosis has been established.
Small cell lung cancer accounts for approximately 15% of lung cancers and has a distinct biology, a distinct treatment approach, and a distinctly worse prognosis than most non-small cell lung cancers. It is strongly associated with smoking, tends to be centrally located, and typically spreads early. Treatment involves chemotherapy and radiation, and increasingly immunotherapy.
Treatment for lung cancer — particularly non-small cell lung cancer with actionable molecular drivers — is one of the most rapidly evolving areas in oncology. New targeted therapies, new immunotherapy combinations, and new data on surgical and radiation approaches are published continuously. The standard of care today may differ from what it was two years ago, and a physician who is not actively engaged with this literature may not be offering the most current options.
The Case for a Thoracic Specialist at a Major Center
For patients with a lung nodule that may be malignant, or with a confirmed lung cancer diagnosis, the case for involvement of a thoracic specialist — a pulmonologist, thoracic surgeon, or thoracic oncologist with specific expertise in lung disease — at a major academic center is strong.
The volume difference matters for procedures. Thoracic surgeons who perform lung resections frequently have lower complication rates and better functional outcomes than those who perform the procedure less often. For a surgery that involves removing part or all of a lung — with permanent consequences for pulmonary function — the experience and volume of the surgical team is directly relevant to the outcome.
The molecular expertise matters for treatment. Centers with active thoracic oncology programs and subspecialty expertise in lung cancer are more likely to have access to the full range of molecular testing, to be aware of the most current evidence on targeted therapies and immunotherapy, and to have clinical trials available for patients whose tumors have specific molecular profiles.
The diagnostic expertise matters for nodule evaluation. A thoracic radiologist who focuses specifically on chest imaging brings a level of pattern recognition to pulmonary nodule evaluation that a general radiologist cannot match. For nodules where the characteristics are ambiguous or the risk assessment is uncertain, subspecialty radiological review can meaningfully refine the clinical picture.

The Patient Who Almost Had a Lung Removed

Not every lung cancer requires the same response. The characteristics of the cancer — its type, its growth rate, its molecular profile, its extent — determine whether surgery, radiation, targeted therapy, watchful waiting, or some combination represents the appropriate course.
A recommendation for lung surgery that cannot be specifically justified by the characteristics of the cancer — that is based on the presence of cancer rather than on the specific features of this cancer — is a recommendation that deserves a second look. The stakes of unnecessary lung surgery are too high, and the alternatives too consequential, for that second look to feel like an optional step.
Questions to Ask When a Lung Nodule or Lung Cancer Diagnosis Has Been Made
For patients navigating a lung nodule finding or a lung cancer diagnosis, the following questions help ensure that the evaluation is complete and the recommendations are appropriate.
For a lung nodule: What are the specific characteristics of this nodule — size, shape, density — and what risk category do current guidelines place it in? What is the recommended follow-up, and how does it compare to Fleischner Society guidelines? What would change the management recommendation?
For a lung cancer diagnosis: What type of lung cancer is this, and has complete molecular testing been performed? What are all of the treatment options for this type and stage? Has this case been reviewed by a multidisciplinary thoracic oncology team? Are there clinical trials relevant to this diagnosis? What is the surgical risk if surgery is being recommended, and how does it compare to non-surgical alternatives?
And in both cases: Has this been reviewed by a thoracic specialist at a high-volume center?
These questions are not obstacles. They are the questions that any informed patient deserves to have answered — and that, in many cases, produce the additional information that changes what happens next.
